**Please review carefully before addressing the following within the online registration system**
I hereby acknowledge that I or my student wishes to attend classes offered by the Ottawa School of Ukrainian Dance (OSUD). I confirm that my student age 12 and over is fully vaccinated with one of the recommended COVID-19 inoculation sets. I acknowledge that the Ottawa School of Ukrainian Dance has provided information on their Covid health and safety procedures including physical distancing measures. OSUD has also taken steps to ensure that all members of OSUD’s teaching team are aware of and will comply with the new protocols. OSUD students and teachers will be asked to confirm their health status each time they enter their designated building (200 Crichton St. Ottawa and/or 35 Stafford Ave. Nepean). I acknowledge and accept that there is a risk that I or my student could be exposed to COVID-19 while attending OSUD activities. In consideration of OSUD agreeing to accept me or my student in classes, I agree to release the Svitanok Ukrainian Dance Society and its officers, directors, instructors, agents and volunteers (the “Releasees”) from any and all causes of action, claims, demands, requests, damages or any recourse whatsoever in respect of any personal injuries or other damages which may occur or arise as a result of exposure to COVID-19 during an OSUD activity.
I do hereby release the Svitanok Ukrainian Dance Society and its respective officers, directors, instructors, agents and volunteers (the “Releasees”) from all claims for loss, injury or damage to persons and property incurred through participation in Ottawa School of Ukrainian Dance activities.
I do hereby acknowledge and agree that notwithstanding the generality of the foregoing, I declare that I will not commence litigation or otherwise seek to recover damages or other compensation against the Releasees based on any action, claim, demand, request, loss or any recourse whatsoever arising from any OSUD activity and/or any potential or actual exposure to COVID-19 while attending OSUD activities and further acknowledge that the Releasees can rely on this Release of Liability and Assumption of Risk as a complete defence to any and all claims, damages, causes of action, or recourse or liability that may arise at any time. I have carefully reviewed this Release of Liability and Assumption of Risk and acknowledge that I fully understand the terms as set out above. I acknowledge that I am signing this Release of Liability and Assumption of Risk voluntarily.
